Tanks	Violations
5	Failed to Empty the Tank to 1" or Less - Per 41 IAC 176.810 (a) (1), when tanks are placed out of service, the tank and all product lines must be emptied immediately. The UST is considered empty when all materials have been removed to one (1) inch or less. Upon reinspection on August 8, 2025, the tanks were stuck using water detection paste applied to the tank stick. Tank 5 was found to contain approximately 5 inches of product. No water was detected.
Facility	Financial responsibility must be reported yearly - Per 41 IAC 176.220, tank owners or operators must submit an electronic Financial Responsibility report form on an annual basis. Financial Responsibility must be maintained until the tank(s) is/are removed or properly abandoned in place by a licensed UST decommissioning contractor.
