Facility Annual Emergency Stop Inspection not completed or inspection report not available. - Emergency shut off switch needs annual testing performed. OSFM form shall be kept on site.
3 Dispensers - hose breakaways are missing or leaking - Hose is missing a breakaway. Install breakaway.

This is for all sumps.
