Submit Notification Form For Ownership Change - The Notification Form is available at https://webapps.sfm.illinois.gov/USTPortal/NotificationForm. Select “New Owner” as the type of notification form. ( Make sure you write down the access Code that is given to you when you start the notification form.) Owner has to match as it appears on the deed. Attach a copy of the deed and also attach a copy of the most recent lease agreement. Attach the lease agreement in the MISC. section. |
