Submit Notification Form For Tank Status Change - Submit OSFM Notification Form placing all tanks in Temporarily Out of Use status.
The Notification Form is available at https://webapps.sfm.illinois.gov/USTPortal/NotificationForm. This form is required to be submitted within 30 days of a tank status change. According to the previous facility manager this tank has been out of service since at least May of 2024 and it received its last fuel delivery on 2/27/2024, per records. |
