Repair or replace equipment. - OSFM received an Overfill Prevention Equipment Inspection form from a licensed UST contractor on 12/17/24, declaring tanks 1 & 2 failed their triennial inspection on 11/26/24. You must repair or replace the overfill devices.
Facility	Financial responsibility must be reported yearly - Per 176.220, tank owners/operators must submit an electronic Financial Responsibility Report form on an annual basis. Financial Responsibility must be maintained until the tank is removed or properly abandoned in place by a licensed UST decommissiong contractor. Warning signs must be seen from all dispensers. 175.220 a) Minimum signage requires letters shall not be less than 7/8” in height. The top of the sign shall be mounted no higher than 10’ above grade. The wording shall include a minimum of 1) “Warning” 2) “Stop Engine” 3) “No Smoking” 4) “Persons fueling vehicles shall remain with their vehicle, at the nozzle, while fueling is in progress” 5) “It is unlawful and dangerous for anyone to dispense gasoline into unapproved containers”

1, 2	Spill prevention equipment contains liquid/debris and must be cleared of materials and maintained clean and dry. - 175.405 a) 1) A) Spill containment equipment that will prevent release of product to the environment when the transfer hose is detached from the fill pipe (e.g., a spill catch basin).
