Failed to Empty the Tank to 1" or Less - Per 175.810(a)(1), when placing tanks out of service, the tank and lines must be emptied
immediately. The USTs are considered empty when all materials have been removed to one inch or
less. Upon my last inspection of the tanks, which was conducted on 6/6/25, I took a stick reading of
each tank and all three tanks had more than an inch of product (tank #1 - 6 inches, tank #2 - 6 inches
and tank #3 - 47 inches). THE PRODUCT MUST BE REMOVED IMMEDIATELY. |

Financial responsibility must be reported yearly - Per 41 Ill. Adm. Code 176.220, tank owners/operators must submit an electronic Financial Responsibility
Report form on an annual basis. Financial Responsibility must be maintained until the tank is removed
or properly abandoned in place by a licensed UST decommissioning contractor. The form can be found

Submit Notification Form For Ownership Change - Per the Cook County Recorder of Deeds office, a Trustee's Deed was recorded on 10/24/07, to 15100
Dixie, Inc. The last notification form filed with our office for a change of ownership was on 4/20/09
declaring "Dixie Inc." as the owner. Our records have to show what appears exactly on the
deed. Therefore, a change of ownership form must be submitted. Here is the link to the electronic

Submit Notification Form For Tank Status Change - Per 175.810(a)(3), tanks taken out of service are required to notify the OSFM within 30 days of date of
last use, on an electronic Notification for Underground Storage Tank form, declaring an out of service
tank status change. Per our findings, the tanks have not been used since 3/1/24. Therefore, you must
