4, 5, 6, 7	Annual leak detection certification not completed (Required annually). - Annual leak detection certification (interstitial sensors/ATG) not completed. Testing shall be completed and submitted to OSFM.
4, 5, 6, 7	Line interstitial sensor functional test not accomplished (Required annually). - STP and UDC sump sensors need annual testing with passing results entered on OSFM portal.
5	Line interstitial sensors have been raised and rendered inoperational. - Regular east STP sump has a raised sensor. Sensor can not be on the manway. Sensors must be in lowest part of sump or per manufactures specifications.
Facility	Miscellaneous - Two exterior Emergency Stop signs must be replaced. Existing signs are faded. 175.210 d) 3) Each emergency stop shall be identified by a sign on all-weather materials stating "EMERGENCY STOP" in 2 inch red capital letters.
Facility	Missing copies of Annual Walkthrough Inspection Report. - Annual Walkthrough by a licensed contractor shall be conducted and documentation maintained on site.
4, 5, 6, 7	Shear valve inspection report not completed (Required during Annual Walkthrough Inspection). - Provide documentation that annual testing of all shear valves has been conducted and passed.
4, 5, 6, 7	Spill prevention equipment contains liquid/debris and must be cleared of materials and maintained clean and dry. - Spill buckets must be kept clean and dry.
4	Stick inserted in drop tube overfill device rendering it inoperative - Remove stick. - Premium drop tube has stick in it. Stick must be removed.
4, 5, 6, 7	Tank interstitial sensor functional test not accomplished (Required annually). - Annual leak detection certification (interstitial sensors/ATG) not completed. Testing shall be completed and submitted to OSFM.
6, 7	Tank interstitial sensors in alarm or failed test, must repair or replace. - ATG is in alarm. L3 and L4 High Liquid Alarm. Repair must be made and alarm cleared. L3 has had a normal sensor report only twice out of 18 months.
5	Water in containment with required interstitial sensors - Remove water and repair containments to prevent water ingress. - Regular east STP sump has liquid in it. Liquid must be removed.
